Documents Required to Register:

Copy of operating authority
Completed and signed Broker/carrier agreement for every carrier.
Valid Tax Identity Number (TIN) completed and signed W-9 tax form
Federal Tax information – submit electronically.
Please email us the above forms.

Certificate of Insurance with the following requirements:

General Liability - $1,000,000
Auto liability – 1,000,000
Evidence of workers compensation or signed waiver (is signed necessary)
Cargo - $100,000

Other Requirements:

Signed and attached carrier packet
Three verifiable references
Evidence of Satisfactory rating with the DOT
